Subtrochanteric Osteotomy in Total Hip Arthroplasty using a Modular Femoral Stem

Abstract

|
Purpose: We wished to analyze the results of total hip arthroplasty (THA) combined with subtrochanteric osteotomy using a modular femoral stem in the cases of DDH sequelae, septic hip sequelae and proximal femoral deformity that are due to previous surgery and fracture
Materials and Methods: 10 patients with 10 THAs combined with subtrochanteric osteotomies were followed for an average 24 months (range: 13~70 months). The mean age was 44.5 years (range: 23~65). We did transverse shortenings and derotational subtrochanteric osteotomies in 9 cases and valgus osteotomy in 1 case. The osteotomy site was fixed with a S-ROM femoral stem in all cases.
Result: The mean femoral shortening was 2.8 cm (range: 0~5 cm). The LLD was improved from 5.2 cm (range: 2.5~8.5) to 2.2 cm (range: 0~4.4). The Harris hip score improved from 35 to 85. 7 preoperative 7 high hip centers were restored to the anatomic hip center. The complications were 2 nonunions and 1 intraoperative diaphyseal crack of the femur. There was no nerve palsy or revision.
Conclusion: The results of THA combined with subtrochanteric osteotomy using a modular stem were satisfactory. However, sometimes an additional fixation is necessary in those cases where the bone quality is poor and the rotational stability is suspicious.
